Job Details

Key Responsibilities:

Meet with patients and families to explain hospice services and guide care transitions

Collaborate with discharge planners, physicians, and hospital staff to identify eligible patients

Provide compassionate education and support to help families make informed decisions

Coordinate hospice admissions and ensure smooth handoffs to the field-based team

Serve as the face of the hospice program on-site at Montefiore
