$81–$87/hour DOE

What You'll Do:

• Provide primary and urgent care in clinic, home, and telehealth settings

• Manage chronic and complex medical conditions

• Lead care planning and clinical coordination efforts

• Conduct annual wellness visits and post-hospital follow-ups

• Collaborate with physicians, specialists, and care teams

• Serve as a clinical leader supporting quality outcomes and patient satisfaction

• Mentor and support clinical staff

Requirements:

• Active NY Nurse Practitioner License

• Active RN License

• Board Certification (AANP or ANCC)

• DEA Registration

• Minimum 2 years of clinical experience

• Primary Care, Internal Medicine, Family Practice, Geriatrics, or related background

• Bilingual Spanish/English REQUIRED

Preferred:

• Experience working with geriatric, frail, or underserved populations

• Leadership or supervisory experience

• Athena EHR experience a plus
