Care Delivery Manager (RN) – Managed Long-Term Care (MLTC)

Location:

New York, NY
Pay Rate: $67.00 - $70.00/hour

Schedule:

Monday - Friday, 8:30 AM - 5:00 PM (1-hour lunch)

Contract:

Temporary opportunity to permanent

We are seeking an experienced Care Delivery Manager (RN) with Managed Long-Term Care (MLTC) experience to oversee care management operations and support the delivery of high-quality, member-centered healthcare services. This leadership role is responsible for supervising clinical staff, ensuring regulatory compliance, coordinating interdisciplinary care, and improving outcomes for a frail and elderly population.
Key Responsibilities

  • Oversee the development and execution of individualized care plans.
  • Facilitate Interdisciplinary Team (IDT) meetings, Service Delivery Requests (SDRs), and related care coordination activities.
  • Provide leadership and support to care management staff while ensuring appropriate staffing coverage.
  • Review clinical documentation, assessments, care plans, and participant charts for quality and compliance.
  • Collaborate with clinicians to ensure accurate UAS assessments and care planning.
  • Manage participant transitions across healthcare settings, including hospitals, rehabilitation facilities, nursing homes, and home care.
  • Investigate incidents, grievances, and complaints while ensuring timely documentation and resolution.
  • Monitor care management activities and member outcomes to ensure effective service delivery.
  • Participate in staff onboarding, coaching, performance evaluations, and professional development.
  • Ensure compliance with all federal, state, organizational, and PACE regulations.
  • Serve as Director of Patient Services (DPS) for the LHCSA when assigned.
  • Participate as an active member of the Interdisciplinary Team (IDT).
Qualifications
  • Active, unrestricted New York State Registered Nurse (RN) license required.
  • Graduate of an accredited Nursing Program.
  • BSN preferred.
  • Completion of NYSED-approved Infection Control coursework required.
  • Minimum 2 years of management or administrative nursing experience in CHHA, LHCSA, acute care, medical-surgical, critical care, nursing home, or related healthcare settings.
  • Minimum 1 year of experience working with a frail or elderly population.
  • Prior supervisory experience required.
  • Strong customer service and leadership skills.
  • Managed Long-Term Care (MLTC) experience strongly required.
  • Case Management Certification preferred within two years of hire.
