Registered Nurse Case Manager
The role requires knowledge of discharge planning and making referrals. The position typically involves working on a Med-Surg or Tele unit and collaborating with a Social Worker. Candidates must be able to function independently and work collaboratively with an interdisciplinary team and patients for discharge planning. Expectations include rounding on the unit with physicians and team members. A positive attitude, critical thinking, and good time management skills are essential.
Utilization Review is handled by a separate department responsible for insurance approvals and arranging transportation. Each floor typically cares for 33-40 patients, with a caseload of 17-25 patients for each case manager. Pilot floors may have additional Social Workers, leading to a lighter caseload of 12-17 patients. Candidates should be capable of handling a minimum of 20 patients. The expected patient care includes discharge to home, community, or skilled nursing facilities.
Duties may involve obtaining housing assistance and coordinating various medical referrals. A resource binder is provided for additional support. Familiarity with Care Port, MCG, and/or Inter Qual is preferred. The position is focused on discharge planning for inpatient populations. Management is supportive of taking designated breaks. Weekend and holiday coverage may be required occasionally, with prior approval from management. Dress code is business casual or any color scrubs except specific restricted colors.
