Director of Nursing - Long Term Care
Location: Bronx, NYSchedule: Full-time, On-site | Monday–Friday | 7:00 AM–3:00 PMCompensation: $180,000 – $210,000/year
She Recruits has partnered with a mission-driven, high-acuity rehabilitation and long-term care center in the Bronx in search of an experienced MDS Director to take the lead on all things assessment, compliance, and care coordination.

Why This Role Rocks:- You'll step into a leadership role with impact—managing a skilled team of 7 MDS nurses while shaping clinical and financial outcomes.
- You'll be the brain behind the facility's MDS strategy, ensuring accuracy, timeliness, and reimbursement optimization under PDPM and Medicaid models.
- You'll collaborate with a strong interdisciplinary team across nursing, therapy, social work, and dietary to ensure every resident's story is captured and supported.
- You'll bring clinical, regulatory, and financial awareness into alignment—and you'll do it with autonomy, support, and recognition.
- Direct the daily operations of the MDS department with full ownership of assessment quality and compliance.
- Lead MDS scheduling, coding accuracy, and documentation integrity to support both resident care and facility reimbursement.
- Analyze quality metrics and CMI data to ensure optimal performance and outcomes.
- Participate in QA committees and lead care plan conferences with residents, families, and clinical teams.
- Keep current with ever-evolving CMS and DOH guidelines to ensure ongoing regulatory readiness.

- NYS RN License (active and in good standing)
- Minimum 5 years of MDS experience
- Proven leadership experience in a long-term care setting (4+ years preferred)
- Expert-level understanding of MDS 3.0, PDPM, and reimbursement models
- RAC-CT certification
