Registered Nurse (RN) in Home Health
Seeking an experienced RN in home health for urgent need. Candidates must be able to review health status questions and make evaluations based on UAS criteria for enrollment. Candidates should demonstrate proficiency with technology in the workplace. Knowledge of the theory, principles, and practices of professional nursing is required. Candidates must be aware of laws and regulations governing professional nursing practices.
Excellent organizational, interpersonal, written, and verbal communication skills are essential. The ability to effectively communicate and work with diverse consumers is necessary. Sensitivity to the concerns of others is important. Candidates should perform comfortably in a fast-paced, deadline-oriented work environment. Ability to manage many complex tasks simultaneously is crucial. Candidates must work independently and demonstrate strong decision-making skills.
Job Responsibilities:
- Perform initial in-home pre-evaluations of patients, including evaluations of health status, strengths, care needs, and preferences.
- Guide the development of individualized long-term care service plans.
- Review consumer medical documentation and health referral forms as relevant.
- Enter evaluation data into electronic evaluation forms as required.
- Document any concerns or conflicting information that arises during evaluations.
- Emphasize continuity of care to reduce fragmentation, duplication, and gaps in treatment plans.
- Discuss health care options, support needs, service vendor options, and waiver options with consumers.
- Initiate communication between eligible consumers and the call center for enrollment into managed care health plans.
- Enhance communication and collaborative relationships with interdisciplinary care team members.
- Collect quality review data and required documentation to support outcome measurements.
- Maintain comprehensive knowledge of community resources, payor requirements, and network services for the target population.
- Meet all established standards for performance and other duties as assigned by management.
