Registered Nurse – Home Health

Job Summary:

Seeking an experienced Registered Nurse for a home health assessment role in Bronx, NY. The RN will conduct comprehensive evaluations, coordinate care plans, and collaborate with interdisciplinary teams to ensure quality patient services.

Responsibilities:

• Perform initial in-home pre-evaluations assessing health status, strengths, care needs, and patient preferences.
• Review medical documentation and health referral forms relevant to each case.
• Enter evaluation data into electronic systems and transmit as required.
• Document concerns, conflicting information, and issues identified during assessments.
• Communicate health care options, supports, service vendors, and waiver options to consumers.
• Collaborate with the Call Center for consumer enrollment in managed care health plans.

Qualifications:

• Hold a current RN licensure in New York State.
• Possess at least two years of recent home health experience within the last 15 years.
• Demonstrate proficiency in conducting patient assessments and working with electronic health records.
• Exhibit strong organizational, interpersonal, and communication skills.
• Be able to work independently in a fast-paced, deadline-oriented environment.
• Sign and adhere to the COI form, providing all required documentation including BLS, NY license, and Nursys verification.
