VIP Community Services seeks an LPN to provide compassionate, community-based nursing care in an integrated health, housing, and social services setting in the Bronx. The LPN will deliver direct patient care, including vital signs, screenings, and medication administration, while supporting primary care, behavioral health, and substance use treatment services. Working within a multidisciplinary team, the LPN will help coordinate care, maintain accurate documentation, and educate clients on health and self-management, contributing to mission-driven, client-centered services for underserved populations.

Responsibilities

• Provide direct patient care under RN/Provider supervision
• Administer medications and treatments per protocols
• Perform vital signs, health screenings, and documentation
• Support primary care and behavioral health services coordination
• Educate patients on health, medications, and self-care
• Assist with care plans and interdisciplinary case conferences
• Maintain accurate electronic health records and reports
• Support community-based and reentry health services
• Follow infection control and safety procedures
• Uphold mission-driven, client-centered care standards
