Responsibilities

• Provide direct patient care to residents in our Skilled Nursing Facility, including administering medications, wound care, and other treatments as prescribed by physicians

• Monitor and report changes in patients' conditions to the nursing team, physicians, and family members

• Collaborate with other healthcare professionals to develop and implement individualized care plans

• Educate patients and their families on self-care techniques, medication management, and disease prevention

• Participate in ongoing quality improvement initiatives to ensure the highest level of patient care
