Licensed Practical Nurse

Bronx Center for Rehabilitation & Healthcare is seeking a Licensed Practical Nurse to provide compassionate care to adult and senior residents in our skilled nursing and rehab facility in the Bronx, NY. The LPN will administer medications and treatments, monitor resident conditions, document care accurately, and collaborate with RNs, therapists, and physicians to support individualized care plans. This role includes direct resident care, wound care support, and health education for residents and families.

Responsibilities

• Provide direct nursing care to adult and senior residents in a skilled nursing and rehab setting
• Administer medications and treatments in accordance with clinical protocols and physician orders
• Monitor, document, and report changes in resident conditions to RNs and physicians
• Support and implement individualized care plans and rehabilitation goals
• Perform basic wound care and assist with restorative and preventive care activities
• Collaborate with an interdisciplinary team including therapists, social workers, and dietitians
• Educate residents and families on care routines, medications, and health maintenance
• Maintain accurate, timely electronic and written clinical documentation
• Promote a safe, respectful, and culturally sensitive environment for residents
• Adhere to all infection control, regulatory, and facility policies and procedures
