Description

The Case Aide Intake Coordinator plays a critical role in the delivery of NYC Preventive Services, serving as the initial point of contact for families referred through the New York City Administration for Children's Services (ACS) and community partners. The position supports family engagement, intake completion, and linkage to concrete services in alignment with Family Connections, OCFS regulations, and NYC child welfare practice standards.

The Case Aide Intake Coordinator works closely with the Program Director, Case Planners, and supervisory staff to ensure appropriate program utilization, accurate and timely documentation, and full compliance with ACS and agency standards. This collaboration supports child safety, permanency, and family well-being while preventing unnecessary foster care placement.

Job Responsibilities:
  • Serve as the initial program contact for families eligible for Preventive Services using trauma-informed, family-centered engagement strategies.
  • Work closely with the Program Director to monitor utilization, ensure intake flow aligns with program capacity, and confirm documentation accuracy and completeness.
  • Complete intake documentation in accordance with ACS, OCFS, and agency timelines and compliance standards.
  • Open Advocate and CWS cases in CONNECTIONS and PROMIS.
  • Gather, verify, and maintain intake data to support auditing, reimbursement, and contract compliance.
  • Support child safety assessments, safety planning, and crisis management efforts.
  • Comply with mandated reporter responsibilities and agency incident reporting procedures.
  • Participate in Family Connections training, coaching, and consultation to maintain model fidelity.
  • Conduct home and office visits and accompany families to mandated appointments.
  • Provide outreach, referrals, advocacy, and escort services addressing concrete needs and housing stability.
  • Obtain collateral documentation from schools, courts, medical providers, and community partners.
  • Carry a limited caseload of up to four (4) cases when operationally required.
  • Utilize mobile technology for real-time documentation and data entry.
  • Attend required supervision, meetings, trainings, and case conferences.
  • Participate in community outreach and agency events as required.
  • Maintain confidentiality and comply with all applicable privacy and data protection standards.
  • Demonstrate cultural humility and professionalism when working with diverse families.
  • Perform other duties as assigned.

Requirements

You must have:
  • Bachelor's degree required;
    Master's degree preferred.
  • 2-5 years of experience in NYC Preventive Services, foster care, or child welfare settings.
  • Knowledge of ACS Prevention Intake processes strongly preferred.
  • Availability for evenings and occasional weekends.
  • Proficiency in Microsoft Office applications.
  • Knowledge of PROMIS and CONNECTIONS or ability to learn quickly.
  • Strong documentation, communication, organizational, and time management skills.
  • Ability to travel within New York City boroughs.
