Osborne has opened its first transitional housing and reentry program, The Fulton Community Reentry Center (Fulton). Fulton will provide 140 beds of transitional reentry housing to formerly incarcerated men who would otherwise be homeless after discharge from a correctional facility. Fulton will also provide onsite reentry services, including case management, benefits enrollment, employment, health, and substance use disorder treatment. The facility operates 365 days on a 24-hour basis daily, including evening and weekend hours (some positions will require non-traditional work hours).

We currently seek a full-time Intake Specialist to join our Fulton Community Reentry Center team! The Intake & Entitlements Specialist serves as a primary point of contact for individuals seeking services at Fulton Community Re-entry Center. This role is responsible for screening, assessing, and admitting participants into appropriate programs while also ensuring eligible clients are connected to critical public benefits, including TANF, SNAP, SSI, and Medicaid.

The Specialist works collaboratively with internal staff, referral sources, and external agencies to ensure timely program admissions, benefit enrollment, and continuity of care for participants, particularly those impacted by substance use and the criminal justice system. This is an on-site position.

Salary Range: $30.00 - $34.00 hourly

Essential Duties:

Intake, Screening, and Engagement

* Serve as the first point of contact for participants seeking services.

* Conduct comprehensive screenings and assessments to determine program eligibility within an 822 setting.

* Facilitate timely intake, admission, and orientation of participants into programs.

* Actively engage participants throughout the intake process to promote retention and program readiness.

* Conduct outreach for missed appointments and follow up with referral sources.

* Participate in case conferences to introduce new participants and coordinate services.

* Maintain accurate client files, intake documentation, and office records.

Entitlements & Benefits Coordination

* Screen all participants for eligibility for public benefits including TANF, SNAP, SSI, and Medicaid.

* Assist eligible clients with benefit applications, recertifications, and follow-up requirements.

* Support clients in obtaining necessary documentation (e.g., birth certificates, Social Security cards).

* Provide individual counseling related to benefits, eligibility concerns, and related barriers.

* Coordinate with DOCCS, Reentry Works staff, and Fulton Community Reentry Center to ensure entitlement screening and access for new participants.

* Perform additional duties as assigned.

Client Education, Referrals, and Outreach

* Orient participants and families to program objectives, expectations, and available services.

* Provide educational interventions related to substance use, recovery, and the impact of addiction on individuals and families.

* Make direct referrals to internal and external services, including residential treatment, medical care, behavioral health, housing, education, and vocational training.

* Maintain ongoing relationships with community-based organizations and external providers to expand service access.

* Assist with outreach efforts to increase program census and connect participants to wraparound services.

Administrative & Reporting Responsibilities

* Enter detailed and timely information into The Osborne Association database.
