Bond Health - Case Manager – Supportive Housing Program

Position Overview

Bond Health is currently seeking a Case Manager to provide residential and long-term support services to formerly homeless and at-risk adults transitioning into permanent housing. The Case Manager will maintain a caseload of approximately 20–25 individuals while providing person-centered case management, community outreach, and ongoing support to promote housing stability and independence.

Key Responsibilities

· Provide comprehensive case management services in both office and community settings.

· Conduct mental health and substance use assessments.

· Develop, implement, and update individualized service plans.

· Assist clients with obtaining public benefits, Social Security benefits, housing resources, and community services.

· Utilize Motivational Interviewing, Harm Reduction, and Critical Time Intervention (CTI).

· Maintain accurate electronic case records and documentation.

· Coordinate referrals to medical, behavioral health, employment, and community-based services.

· Facilitate life skills, education, support, relapse prevention, and wellness groups.

· Serve as a liaison between clients and community providers.

· Collaborate with Workforce Development staff to support employment-related goals.

· Conduct outreach, apartment visits, inspections, and follow-up services.

· Assist tenants with budgeting, lease compliance, and landlord-tenant mediation.

· Utilize trauma-informed and culturally responsive practices.

· Provide crisis intervention and ongoing support.

· Ensure compliance with program and funding requirements.

· Perform other duties as assigned.

Please Note:

Community-based field work may account for approximately 85% of this position.

Qualifications

· Bachelor's degree in Human Services or a related field required.

· Experience working with homeless, low-income, or vulnerable populations preferred.

· Strong organizational, communication, documentation, and computer skills.

· Ability to work independently in the community.

· Fluent Spanish is strongly preferred.

· Self-motivated with excellent interpersonal and problem-solving skills.
