Bond Health - Care Manager / Case Manager – Family Supportive Housing

Position Type:
Full-Time | Temp-to-Perm

Position Overview

Bond Health is currently seeking a Care Manager / Case Manager to support formerly homeless families participating in a Family Supportive Scatter-Site Housing Program. The Care Manager will maintain a caseload of approximately 15 families and provide person-centered case management services through home visits, community outreach, and ongoing support.

Key Responsibilities

· Provide comprehensive case management services to formerly homeless families.

· Conduct psychosocial, mental health, and substance use assessments.

· Develop, implement, and update individualized service plans.

· Assist clients with obtaining benefits, housing resources, and community services.

· Maintain regular contact through home visits, meetings, phone calls, and follow-ups.

· Utilize Motivational Interviewing, Harm Reduction, Critical Time Intervention (CTI), and trauma-informed care.

· Maintain accurate electronic documentation.

· Coordinate referrals to medical, behavioral health, educational, employment, and community services.

· Facilitate life skills, support, and educational groups.

· Conduct outreach, apartment visits, and housing stability follow-up.

· Assist with budgeting, financial counseling, and ADL support.

· Provide crisis intervention and collaborate with interdisciplinary teams.

· Ensure compliance with program and funding requirements.

· Field work may account for approximately 85% of the position.

Qualifications

· Bachelor's degree in Psychology, Social Work, Human Services, Child Development, or a related field.

· Minimum two years of experience working with individuals with severe mental illness.

· Minimum two years of experience working with children with developmental needs.

· Experience working with homeless or vulnerable populations preferred.

· Knowledge of Housing First, Harm Reduction, and Motivational Interviewing.

· Strong organizational, communication, documentation, and computer skills.

· Experience with Foothold AWARDS is a plus.

· Bilingual Spanish/English preferred.
