Position: CASE MANAGER (Transitional)
Case Manager
The Case Managers are responsible for performing a variety of tasks and activities that are designed to effect the lasting rehabilitation of residents including; initial and ongoing assessment of residents; monitoring and evaluation resident level of functioning; development and review of individualized service plans with residents; and delivery of supportive services including harm reduction in accordance with the resident's need, desires and goals and for acting as an advocate for the resident.
Case managers are also responsible for working as a member of the service team and for having a working knowledge about the epidemiology of AIDS, clinical drug trials, CPR, First Aid, infection control and OSHA guidelines.
Case Managers maintain an active case load of single adults.
This is an exempt position.
Responsibilities include but are not limited to:
Participate in the intake for admission to the residence program.Conduct an assessment of residents within required time frames to determine resident functioning, needs and strengths.Develop initial individualized service plan with residents in accordance with the resident's desires, needs and strengths. Involve resident's support system and other social service providers as necessary and with the involvement of resident.Review service plans and revise as necessary with residents, the resident support system and collaterals.Develop an individual case management service plan with residents, other program staff, the resident's support system and other support services staff and providers.Ensure that the objective and services/methods included in the service plan and case management plan relate to the goals established by staff and the residents, and that objectives and services are relevant to the resident. Provide skills training as necessary and in accordance with the resident's desires, needs and goals.Record the progress of residents within 24 hours of service provision and in accordance with the support service plan and case management plan. Record progress of residents within 24 hours of contact with the resident that is related to the individualized support service and case management plans. Record telephone conversations with other providers immediately following each telephone contact.Insure that resident case records are current and up-to-date and that they are maintained in accordance with HASA and HOGAR requirements and legal entitiesMake referrals of residents to locally based support service providers as required, act as liaison with other service providers, escort residents to meetings and appointments with other support service providers as needed and maintain regular contact with other support services providers to ensure continuity of care.Respond to resident crisis, emergencies and other situations that require immediate staff involvement and action.Encourage resident participation in group social and recreation activities, and participate in such activities as required.Maintain a positive working relationship with residents and encourage resident participation in on and off-site programs.Maintain a positive working relationship with other support service providers.Attend conferences, meetings, and educational seminars as required.Act as an advocate for residents.Maintain the confidentiality of residents at all timesInterviews incoming clients relative to their needs for income assistance.Assess the income and income assistance needs of clients and develops an entitlement assistance plan, where necessary, which will best serve the client's needs and which is consistent with rules, regulations and guidelines of applicable benefit programs.Reviews benefit and income assistance programs to ascertain requirements and application process.Brings to the attention of the staff any information that would be relevant to the client's treatment and ability to pay.Advises clients of requirements of financial assistance program(s) to which the client may be entitled and the application process.Assists clients in completing relevant applications to insure that appropriate financial assistance is accessed by the client.Schedules…
