Case Manager
As a Case Manager, you'll play an important role in supporting older adults by connecting them with essential benefits, services, resources, and programs. You'll provide Information and Referral and Case Assistance services both in person and remotely, helping participants navigate available resources and advocating for their needs so they can maintain their health, independence, and quality of life.
Under the supervision of the Program Director of the Older Adult Center, the Case Manager provides Information and Referral and Case Assistance services to participants at the center, both in person and remotely.
The Case Manager assesses participant needs, provides information about available services and benefits, assists with applications and referrals, advocates on behalf of participants, and helps connect older adults with appropriate community resources.
Information, Referral & Case Assistance- Provide participants with information about health, welfare, education, social, recreational, and other services available to older adults.
- Assess and screen participant eligibility for appropriate benefits and entitlements.
- Assist participants with applications for benefits and entitlements.
- Advocate on behalf of participants to help secure appropriate benefits and services.
- Explain and interpret JASA programs and available services to participants.
- Assist participants with arranging and accessing services, including transportation, homecare services, home-delivered meals, public assistance, Medicare and Medicaid, emergency cash relief, legal services, protective services, vocational placement, medical and psychiatric examinations and therapy, housing, and other appropriate community-based services.
- Work closely with community organizations and service providers to address participant needs.
- Help participants navigate available resources and advocate for appropriate services.
- Conduct community education activities and support groups.
- Provide guidance and assistance to participants experiencing challenging circumstances.
- Inform the supervisor of crisis situations or circumstances requiring additional guidance or intervention.
- Participate in trainings, unit meetings, and other professional development activities.
- Promote awareness and understanding of JASA programs and services.
- Maintain complete and accurate case records.
- Prepare required reports and statistics.
- Utilize secure electronic databases and program-specific software as required.
- Maintain confidentiality and appropriate documentation of participant services.
- Perform other related duties and assignments as requested.
- Participate in coverage of "Cooling Center Hours," "Make-Up Days," and other necessary responses for older adult center members when ordered by City funders.
- Be available to work extended hours during the week or weekends when required for emergency operations or other necessary program responses.
- Graduate of an accredited college or university with a B.S.W., B.A., or B.S. degree.
- Prior experience providing case management, social services, information and referral, benefits assistance, or related services is preferred.
- Some prior group leadership experience is preferred.
- Experience working with older adults or within a community-based social services environment is a plus.
- Strong oral and written communication skills, including effective record-keeping abilities.
- Ability to read, write, speak, and understand English.
- Computer literate, with the ability to learn and utilize program-specific software and secure electronic databases.
- Strong interpersonal and organizational skills.
- Ability to communicate effectively with older adults, families, community organizations, and service providers.
- Ability to work independently while also functioning effectively as part of a team.
- Strong advocacy, problem-solving, and resource-navigation skills.
- Ability to recognize crisis situations and seek appropriate supervisory guidance.
