Housing Case Manager

Compensation Range: $24/hr - $31.25/hr commensurate with experience

Overview:
The Housing Case Manager (HCM) provides comprehensive Case Management services and care coordination to a caseload of residents who are living with HIV and/or co-occurring conditions. The Housing Case Manager works closely with their clients in the clients' homes, in the office, and in the community helping to ensure housing stability by promoting independent living skills, financial management, and health and wellness.

Responsibilities:

  • HCM provides case management services to assigned clients, including supportive counseling, motivational interviewing, crisis intervention, care coordination, linkage and referral, assessment and service plan development, fieldwork, and assessment/observation of resident in their living space as needed but no less than once per month.
  • Documentation of all services and interventions provided as per funder, regulatory, industry, and internal standards.

Key Performance Indicators (KPIs):

  • Successfully complete monthly home visits for 100% of your clients.
  • Ensure that HIPAA forms, and income verification documents are uploaded into each client's file and are signed if applicable.
  • Work with your clients to create comprehensive initial assessments and service plans within 30 days of enrollment, and reassessments and service plans every 90 days

Other functions that may be needed with this position include:

  • Provide referrals, and follow-up, when necessary, for primary care, substance use services, nutrition, clothing, mental health services, and financial management.
  • Maintain meaningful engagement with residents, using motivational interviewing and other supportive interventions to help residents achieve and maintain the highest level of independent functioning possible.
  • Promote participation in community activities and other endeavors that promote individual growth and development of a social support network.
  • Utilizing the vast array of internal services available to clients, look to connect (and coordinate) with other departments including: ADHC, Health Services, OASAS, Health Homes, Behavioral Health, Community Partnerships, Legal, and Job-Training.
  • Help new clients move into their apartments and assist clients with their transition from homelessness to stable housing.
  • In collaboration with the client, develop strength based individualized goals. Provide constructive feedback on goal participation during scheduled engagement.
  • Accompany residents as needed to facilitate access to external services.
  • Assess and discuss financial management with the client, including following up with the client on any rent obligation, maintaining an active subsidy, consistently making utility payments, and food and nutritional needs.
  • Complete apartment inspections and place work orders as needed, ensuring collaboration with the custodial and operations staff.
  • Actively participate in weekly supervision with your supervisor. Come to the meetings prepared.
  • Complete all required reports and paperwork by the prescribed deadlines.
  • Demonstrate excellent customer service with clients, staff, vendors, and all other relevant stakeholders.
  • Coordination of care, including but not limited to case conferencing, with residents' internal and external service providers. Participate in case conferences, internally or externally, as part of a multidisciplinary team.
  • Perform other responsibilities and duties as assigned.
  • Enthusiastically take part in advocacy and direct actions to end the twin crises of homelessness and HIV/AIDS

Requirements:

  • Bachelor's degree in social work, psychology, counseling, or a related field preferred. Master's Degree a plus.
