Medical Practice LPN

LPN Position – Primary Care Geriatric Medical Program

40 hours per week, Monday‑Friday, 8‑hour shifts between 7:45 am and 5:30 pm.

Summary: Assist the clinician in providing medical care to patients of the Medical Practice.

• Prepare materials, supplies, equipment, and patient for medical exams.
• Obtain patient history and medical information.
• Provide direct patient care as directed by a registered nurse or clinician.
• Collect and process specimens, perform point‑of‑care testing as directed.
• Administer medications and vaccinations, instruct patients and families on preventive care, medication intake, test preparation, and monitoring treatment progress.
• Contact patients to provide follow‑up, advise of test results, or relay clinicians' instructions.
• Fulfill and adhere to all compliance responsibilities related to the position.
• Maintain and model the organization’s values.
• Demonstrate regular, dependable, and predictable attendance.
• Perform other duties as required.

• Ability to travel between practices as needed.

• Active LPN license based on state.

• BLS certification required within 30 days of hire.

• Manual:
  Little or no manual skills, motor coordination, or finger dexterity.
• Occupational:
  Little or no potential for occupational risk.
• Physical Effort:
  Sedentary/light effort; may exert up to 10 lbs.
• Physical Environment:
  Generally pleasant working conditions.

Salary Range: $26.48 – $50.49 hourly.
