Medical Practice LPN

Northwell Health seeks a Medical Practice LPN to support outpatient patient care within a collaborative, patient-first environment. The LPN will room patients, obtain vitals, perform injections, ECGs, and point-of-care testing, assist with minor procedures, manage phone triage and refills per protocol, and accurately document in the EHR. This role supports care coordination, patient education, and follow-up to ensure high-quality, compassionate service.

Responsibilities

• Room patients and obtain vitals and medical history
• Administer injections, vaccines, and point-of-care tests
• Assist providers with minor procedures and treatments
• Perform ECGs and specimen collection per protocol
• Provide patient education and discharge instructions
• Manage phone triage, refills, and patient messages
• Document accurately in EHR and maintain chart quality
• Support care coordination and follow-up appointments
• Maintain infection control, safety, and equipment readiness
• Collaborate with multidisciplinary team to improve workflows
