Travel RN Home Health - Brookfield, WI

Schedule & Hours:

• 13-week contract
• Day shifts, 10 hours (8:00 am - 6:00 pm)
• Shift options:
  Thurs/Fri/Sat/Sun or Sat/Sun/Mon/Tues
• 40 guaranteed hours per week (except during orientation week and holidays)
• No on-call requirements

Required Experience & Qualifications:

• Active WI RN license & American Heart BLS
• 1-2 years of recent home health and hospice nursing (admissions, symptom management, end-of-life care)
• Proficiency with Oasis and Epic charting platforms
• Valid driver's license; comfort with significant daily travel (50-100 miles)
• Strong time management, resilience, adaptability, and independence
• Ability to manage documentation efficiently and deliver care in diverse home settings
• COVID-19 vaccination or approved exemption

Primary Responsibilities:

• Provide high-quality, compassionate care to adults with chronic and complex conditions in their homes
• Conduct home health admissions, symptom management, hospice visits (including end-of-life care)
• Collaborate with interdisciplinary care teams and ensure timely, thorough documentation
• Promote patient education, wellness, and safety during all visits
