Admissions/Discharge Registered Nurse (RN)
Complete Care at Care Age is seeking an experienced Admissions/Discharge Registered Nurse (RN) to welcome residents and lead the transition process. As our Admissions/Discharge Registered Nurse (RN), you serve as a calm guide during key transitions without daily floor or med pass responsibilities.

Key Responsibilities
Admission & Clinical Care:
Perform unit duties daily to complete admissions, verify orders, review labs, and deliver excellent customer service to new admissions.Leadership & Consultation:
Review admissions, communicate findings to the Unit Manager or Director of Nursing (DON), and consult with the DON on facility capabilities and admission decisions.Order Management & Documentation:
Transcribe provider orders, query providers for accuracy, and complete or direct all required admission documentation.Discharge Coordination:
Assist with discharge planning, call in prescriptions, coordinate paperwork, and communicate with families and social services to ensure smooth transitions.Payor Knowledge:
Maintain working knowledge of facility practices across all payor types (Medicare A, Medicaid, Private, commercial contracts, hospital contracts).Clinical Support & Emergency Response:
Assist with labs, resident status changes, and charting as needed (Charge Nurse duties as time permits); manage emergent situations/falls, including assessments, documentation, provider/management communication, and transfer/bed hold procedures.
Specific Skills/Experience
REQUIRED:
Associate degree of nursing.
Current Wisconsin Registered Nurse licensure.
Current CPR/BLS certification.
Must have no disciplinary action in effect against licensure.
Ability to work under pressure.
Specific Skills/Experience PREFERRED:
Previous experience in long-term care preferred.
Familiarity with Point Click Care preferred.
