JOB TITLE:

Certified Nursing Assistant (CNA) / Home Health Aide (HHA) Home Health & Hospice

STATUS: Full-Time Non-Exempt

REPORTS TO: Director of Nursing / Clinical Supervisor

LOCATION: Brookings, OR with travel to Oregon and California patient homes

POSITION SUMMARY

The CNA/HHA is a paraprofessional member of the interdisciplinary care team who provides personal care and supportive services to patients in accordance with the physician-ordered plan of care and hospice plan of care. The aide works under the supervision of a Registered Nurse and performs services necessary to meet the patient’s personal needs, promote comfort, and support quality of life. Care is delivered in patient homes across Oregon and California in accordance with applicable state regulations and federal Conditions of Participation.

ESSENTIAL DUTIES AND RESPONSIBILITIES Patient Care
  • Provide personal care and activities of daily living (ADLs) as assigned.
  • Assist with bathing, grooming, oral hygiene, skin care, and dressing.
  • Assist with toileting, transfers, ambulation, and positioning.
  • Take and record vital signs when ordered and competency validated.
  • Observe patient condition and report changes promptly to the supervising nurse.
  • Document care provided on the day of service per agency policy.
Home Support
  • Maintain a clean, safe patient environment.
  • Perform light housekeeping related to patient care.
  • Provide respite support for caregivers when assigned.
Communication & Teamwork
  • Communicate effectively with RN/Case Manager and interdisciplinary team.
  • Participate in case conferences and care coordination as requested.
  • Maintain patient confidentiality in compliance with HIPAA.
  • Provide compassionate, respectful care aligned with hospice philosophy.
Compliance & Professional Standards
  • Follow infection control, safety, and emergency preparedness protocols.
  • Adhere to delegation guidelines and scope-of-practice laws.
  • Participate in required in-services, competencies, and QAPI activities.
  • Maintain professional boundaries and ethical conduct at all times.
JOB LIMITATIONS
  • Will not administer medications (except permitted reminders per state law).
  • Will not take physician orders.
  • Will not perform sterile procedures.
  • Will not perform any task requiring licensed nursing judgment.
  • Will not function outside state aide scope of practice.
QUALIFICATIONS Required
  • Current Certified Nursing Assistant (CNA) on Oregon registry and California registry (or ability to obtain CA within 90 days).
  • Current Home Health Aide (HHA) competency meeting federal requirements.
  • Minimum age 18.
  • Current CPR certification (BLS preferred).
  • Valid driver’s license and reliable insured vehicle.
  • Ability to read, write, and document in English.
  • Ability to work independently in home settings.
  • Empathy and comfort working with seriously ill and hospice patients.
Preferred
  • California Home Care Aide (HCA) registration.
  • Prior home health or hospice experience.
PHYSICAL REQUIREMENTS

Employee must be able to lift, transfer, and reposition patients with assistance as needed; perform frequent standing, walking, bending, and reaching; drive in varying weather and road conditions; use visual and hearing acuity for patient care and documentation; and tolerate exposure to blood and body fluids. Reasonable accommodations will be provided in accordance with applicable law.

  • Community home settings in Oregon and California.
  • Exposure to pets, smoke, clutter, and variable home conditions.
  • Possible exposure to infectious diseases.
  • Frequent automobile travel required.
  • Typical hours Monday–Friday with occasional weekends/holidays as needed.
