Insight Global's client is seeking a Physical Therapist Assistant (PTA) to provide skilled therapy services to patients in the home health and hospice setting under the direction of a licensed Physical Therapist and physician plan of care. This position focuses on helping patients improve mobility, maximize independence, reduce functional limitations, and enhance overall quality of life in the comfort of their homes.
The ideal candidate is compassionate, patient-focused, and comfortable working independently while collaborating closely with therapists, physicians, patients, families, and interdisciplinary care teams.
Must Haves
- Associate degree from a Physical Therapist Assistant program approved by the American Physical Therapy Association (APTA)
- Active Oregon Physical Therapist Assistant license (or willing to transfer)
- Minimum 1 year of PTA experience
- Valid driver's license and reliable transportation
- Ability to provide treatment under the supervision of a Physical Therapist
- Strong communication and patient education skills
- Documentation and care planning experience
- Ability to work independently in a home-based care environment
Nice to Haves
- Home Health experience
- Experience working with geriatric patients
- Knowledge of Medicare and home health documentation requirements
- Experience participating in interdisciplinary care teams
- Experience with performance improvement or quality initiatives
- Comfort educating patients and family members on home exercise programs and activities of daily living
Day to Day
- Provide physician-prescribed physical therapy treatments under the plan of care established by the Physical Therapist
- Perform therapy interventions aimed at improving mobility, function, and independence
- Help patients minimize physical limitations and achieve their highest level of functional ability
- Collaborate with Physical Therapists and other healthcare professionals on patient care plans
- Communicate patient progress and treatment concerns to the supervising Physical Therapist
- Educate patients and family members on home exercise programs, safety, and activities of daily living
- Participate in care planning meetings, patient care conferences, and interdisciplinary team discussions
- Complete visit documentation, progress notes, and care plan updates in a timely manner
- Assist with clinical documentation and patient progress reporting
- Participate in agency training, in-service education, and performance improvement activities
- Follow agency policies, procedures, and regulatory requirements
- Travel to patient homes throughout the assigned service area to provide care and treatment
