Compensation Range:

• Without certification - $21.43 - $32.42.
• With certification - $22.46 - $33.97.
  • Compensation is based on experience, with a range reflecting entry-level candidates through those with 15+ years of experience. Eligible positions may also qualify for additional compensation with applicable shift, weekend, certification, or other differentials.

MINIMUM JOB REQUIREMENTS

Education & Experience

• High School graduate or equivalent is required.
• Certified Clinical Medical Assistant through the National Health careers Association (CCMA) required.
  • For those with at least 6 months of prior work experience as an MA, or a certificate of completion from a Medical Assistant program, CCMA certification must be obtained within 6 months of employment with CHN;
  • For those with less than 6 months of experience as an MA, CCMA certification must be obtained within 1 year of employment as an MA with CHN.
• Current AHA BLS certification is preferred at the time of hire or must be obtained within 60 days of employment.
• Successful completion of a Medical Assistant Program preferred.
• Six months or more Medical Assistance experience preferred.

Required Knowledge, Skills & Abilities

• Basic computer knowledge including Microsoft Word, Excel and Outlook is required.
• Good communication skills are required, both verbally and in written form.
• Excellent customer service skills are required.

Additional Compensation Differential Opportunities:

• Licensed Positions: NOC Shift (7:00 PM–7:00 AM): +$3.50/hour. | Weekend Shift (12:00 AM Saturday–11:59 PM Sunday): +$3.00/hour.
• Non-Licensed Positions: NOC Shift (7:00 PM–7:00 AM): +$3.00/hour. | Weekend Shift (12:00 AM Saturday–11:59 PM Sunday): +$2.00/hour.

JOB SUMMARY

Under the general supervision of the Director of Primary Care and Population Health, The Population Health Medical Assistant (PHMA) supports the Population Health team and primary care clinics in improving patient outcomes through proactive outreach, care-gap closure, clinical data review, and coordination of preventive and chronic disease care. This position works closely with providers, nurses, care coordinators, and other members of the care team to identify patients who may benefit from additional services and ensure appropriate follow-up.

The Population Health MA uses clinical knowledge, organizational skills, and attention to detail to support population health initiatives, quality improvement programs, and value-based care goals across the organization

ESSENTIAL FUNCTIONS

Review patient panels, registries, reports, and medical records to identify preventive, wellness, and chronic care needs; support care gap identification, chart review activities, pre-visit planning, and quality data validation.

Conduct proactive patient outreach through approved communication methods to facilitate preventive care, chronic disease management, completion of recommended screenings and testing, wellness visits, immunizations, and other follow-up services.

Coordinate patient care activities by scheduling appointments, laboratory services, screenings, referrals, and follow-up care, while supporting timely access to appropriate clinical and community resources.

Support population health and chronic disease management programs by assisting with patient education, reinforcing established care plans within the Medical Assistant scope of practice, and facilitating communication with providers, nurses, care coordinators, dietitians, and other members of the care team.
