Hospice RN Case Manager Salary & Compensation- Starting Rate: From $44.00 per hour (Highly negotiable; starting pay is greatly influenced by your depth of experience).

We are seeking a permanent, dedicated Hospice RN Case Manager to join our compassionate care team in Brookings, Oregon
. In this role, you will provide end-of-life care, symptom management, and emotional support to patients and their families. Because our service area spans counties on both sides of the state line, this position uniquely services communities in both Southern Oregon and Northern California
.

- Clinical Assessment: Manage complex physical symptoms, administer medications, and provide skilled nursing interventions.

Work alongside physicians, social workers, spiritual counselors, and home health aides to deliver holistic care. - Documentation: Maintain accurate, timely electronic charting regarding patient status and care plans.
- Family Support: Provide vital education, emotional reassurance, and guidance to families and caregivers.
- Experience: Verifiable, professional hospice nursing experience is strictly required
. - Licensure: Must hold active, unrestricted Registered Nurse (RN) licenses in both Oregon and California (or have immediate eligibility to practice in both states).
- Employment Type: Looking for permanent
, long-term team members. - Transportation: Valid driver’s license, reliable vehicle, and clean driving record for traveling to patient homes.
