Licensed Practical Nurse - DSL

Role Overview

The Licensed Practical Nurse (LPN) administers skilled nursing care to patients in their homes, following physician orders and the care plan under supervision. Responsibilities include implementing nursing procedures, observing patient conditions, reporting reactions, educating patients and families, maintaining accurate records, and participating in team meetings and care conferences. The role requires adherence to policies, proper documentation, and collaboration with healthcare team members.

• Licensed in Oregon with graduation from an approved practical nursing program
• Current BLS certification
• Minimum one year of relevant experience; three years preferred
• Knowledge of nursing techniques, medical terminology, medication effects, and recordkeeping
• Ability to interpret orders, work independently, and communicate compassionately
• Must possess a valid driver’s license, insurance, and maintain a good driving record

• Ability to perform physical tasks, including lifting and mobility
• Effective stress management and adaptability to various home environments
• Travel within service area necessary
• Work may include evenings, holidays, and on-call duties
