The Licensed Practical Nurse (LPN) role in Home Health delivers skilled nursing care to patients in their residences under physician orders and care plans. You will monitor symptoms, administer procedures, document updates, and educate families.
You must be licensed in Oregon, hold current BLS certification, and have at least one year of home health experience (three years preferred). A valid Oregon driver’s license and reliable vehicle are required to reach patients.
