Wound Care Nurse (RN or LPN)
Job Summary:
Palisade Healthcare Center is seeking a compassionate and experienced Wound Care Nurse (RN or LPN) to lead and manage wound care services for our residents. In this role, you will assess, develop, and implement individualized wound treatment plans while collaborating with the interdisciplinary team to promote healing and improve resident outcomes.
Essential Position Duties:- Assess, evaluate, and treat residents' wound care needs.
- Develop, implement, and monitor individualized wound care treatment plans.
- Supervise and support nursing staff in providing quality wound care.
- Ensure compliance with all state and federal regulations related to wound care and nursing services.
- Collaborate with physicians, therapists, dietitians, and other members of the healthcare team to optimize resident care.
- Maintain professional nursing standards and accurate clinical documentation.
- Provide support to the Director of Nursing as needed.
- Review resident care concerns and assignments with the Director of Nursing and Administrator.
- Observe changes in resident condition, needs, or preferences and communicate them promptly to the Director of Nursing and/or Resident Care Manager.
- Demonstrate knowledge of and follow facility policies regarding fire safety, disaster preparedness, infection prevention, workplace safety, and emergency procedures.
- Receive and carry out written and verbal physician orders in accordance with state regulations.
- Required:
Current RN or LPN license in the State of South Dakota in good standing. - Preferred:
Current wound care certification. - Minimum of one (1) year of wound care experience in a skilled nursing, long-term care, rehabilitation, or healthcare setting preferred.
- Training or experience in rehabilitative and restorative nursing practices preferred.
- Thorough knowledge of nursing practices, wound care principles, and infection prevention.
- Knowledge of state and federal regulations governing skilled nursing facilities.
- Strong assessment, critical thinking, and documentation skills.
- Excellent communication and teamwork abilities.
- Ability to educate staff, residents, and families on wound care best practices.
