Home Health Physical Therapist (Brookline, Massachusetts)

Full‑time Home Health Physical Therapist position to provide exceptional rehabilitative therapy to geriatric patients in senior living communities in Cleveland Circle. New graduates are encouraged to apply.

• Make visits to clients in senior living communities (Assisted Living and Independent Living Communities).
• Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance, balance testing and safety assessments.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance and other interventions.
• Continuously assess and revise the PT care plan and collaborate with nursing on multidisciplinary care planning.
• Educate and instruct clients, family members, or representatives on rehabilitative care and activities to promote health, safety and independent living.
• Accurately document observations, interventions, and evaluations using state‑of‑the‑art tablet technology.

• Current license as a Physical Therapist in Massachusetts.
• Prior home health experience is not required.

$65.00 - $75.00 per point depending on qualifications.
