Home Health Aide Per Diem

Hebrew Senior Life -  - Responsibilities:
Provide personal care services to Hospice and Home Care patients as prescribed by the physician;
Assist with activities of daily living including baths, mouth/denture care, grooming, transferring, dressing, ambulation, and turning for comfort;
Assist with household tasks directly essential to the patient's health and environment;
Assist the patient in self-administering medications and remind them to take medications as directed;
Transport to patient's home/facility as needed and maintain professional relationships with patients and families
