The Opportunity

Boost Home Healthcare is a Medicare-certified, ACHC-accredited agency in Brookline. We are looking for an exceptional RN Case Manager to join our ground-floor team. If you want supplemental income without the red tape of a large corporation, this role is built for you.

What you’ll be doing
  • Primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.
  • Conduct initial evaluations and develop individualized plans of care.
  • Complete accurate OASIS assessments and timely documentation.
  • Coordinate care planning with physicians and our clinical team.
  • Provide education, resources, and emotional support to patients and their families to promote independence and wellness.
What we’re looking for
  • Licensure: RN (Registered Nurse) with an active MA state license.
  • Experience: At least 1 year of experience in home health or case management preferred. Expertise in administering IVs and medications.
  • Reliability: Valid MA driver’s license and reliable transportation.
  • Availability: Able to commit 10 - 20 hours per week, flexible hours.
  • Knowledge: Familiarity with Medicare OASIS (training is available).
  • Skills: Strong critical thinking, communication, time management, and the ability to work independently.
  • Tech‑Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or Well Sky is a plus).
  • Reliable Transportation: Valid driver’s license, auto insurance, and access to a dependable vehicle.
