Position SummaryThe Assisted Living Coordinator supports the Licensed Assisted Living Director (LALD) and CEO in overseeing daily operations, coordinating resident care and services, ensuring regulatory compliance, and supporting facility growth.
This role is critical in both ongoing operations and the development, preparation, and licensing of new assisted living facilities under Minnesota Department of Health (MDH) requirements. The Coordinator also leads community outreach, marketing, and admissions efforts to support occupancy and organizational growth.
Key Responsibilities Resident Care & Service Coordination- Coordinate health-related and supportive services in alignment with individual Service Plans
- Assist with resident admissions, assessments, and reassessments
- Communicate effectively with residents, families, and staff to address concerns
- Maintain accurate, compliant resident records
- Assist with staffing coordination and scheduling
- Support onboarding, training, and performance monitoring of direct care staff
- Ensure staff adherence to policies, procedures, and quality standards
- Participate in staff meetings and team collaboration
- Support compliance with Minnesota Statute 144G, MN Rules 4659, and MDH standards
- Assist with licensing and setup of new assisted living facilities, including MDH inspections and documentation
- Participate in quality audits, QAPI activities, and inspections
- Ensure completion of required documentation and incident reporting
- Coordinate inquiries, tours, and admissions
- Build and maintain referral relationships with hospitals, case managers, social workers, and community partners
- Represent Fortunate Homes LLC at community events and networking opportunities
- Track referrals, inquiries, and move-ins to support occupancy goals
- Serve as a liaison between residents, families, staff, and leadership
- Communicate operational or resident-related issues promptly to leadership
- Support cross-department communication and coordination
- Associate’s or Bachelor’s degree in Health Care Administration, Human Services, Marketing, Nursing, or related field (preferred)
- Minimum 2 years of experience in assisted living, senior housing, home care, or healthcare administration
- Knowledge of Minnesota Assisted Living regulations (144G) and MDH licensing standards
- Strong organizational, communication, leadership, and customer service skills
- Marketing, business development, or assisted living start-up experience strongly preferred
- Ability to manage multiple priorities, work independently, and maintain confidentiality
- Part-time with potential transition to full-time
- Primarily on-site with flexibility for resident, marketing, or staffing needs
- Occasional evenings, weekends, and community events
- Travel between Brooklyn Park, Brooklyn Center, and Plymouth locations as needed
- Ability to stand, walk, and occasionally lift up to 25 lbs
