About the Role

As the Assistant Nursing Director, you will provide clinical leadership while overseeing the day‑to‑day nursing operations within our assisted living community. You will work closely with leadership, physicians, families, and direct care staff to ensure residents receive exceptional care that supports both their physical and mental health needs.

This position combines hands‑on clinical care with leadership responsibilities and offers the opportunity to make a meaningful difference every day.

Key Responsibilities Leadership & Staff Development
  • Provide leadership, mentorship, and clinical guidance to nursing and direct care staff.
  • Promote a positive, accountable, and resident‑centered culture.
  • Assist with hiring, onboarding, coaching, and evaluating nursing staff.
  • Coordinate orientation and ongoing education for caregivers, medication aides, and nursing personnel.
  • Support staff development through mentoring and clinical training.
  • Assist with scheduling and departmental staffing needs.
Resident Assessments & Care Planning
  • Complete pre‑admission nursing assessments to determine resident appropriateness for admission.
  • Perform comprehensive nursing assessments upon admission.
  • Conduct quarterly resident assessments and annual evaluations.
  • Complete assessments related to significant changes in condition.
  • Develop, implement, and update individualized service plans.
  • Lead interdisciplinary service plan meetings.
  • Ensure resident care plans accurately reflect current needs, preferences, and physician orders.
  • Monitor resident progress and adjust care plans as needed.
Clinical Oversight
  • Provide direct clinical oversight of resident care.
  • Monitor residents for changes in condition and initiate appropriate interventions.
  • Oversee medication management and nursing services.
  • Coordinate physician orders and ensure timely implementation.
  • Review nursing documentation for completeness and accuracy.
  • Collaborate with home health, hospice, therapy providers, pharmacies, and other healthcare professionals.
  • Assist with wound care oversight, infection prevention, and chronic disease management.
Care Management & Collaboration
  • Partner with physicians, nurse practitioners, pharmacists, families, and interdisciplinary team members to coordinate resident care.
  • Communicate resident status changes promptly.
  • Participate in discharge planning and care transitions.
  • Support residents experiencing mental health concerns through collaborative care planning.
  • Foster positive relationships with residents, families, and outside healthcare providers.
Quality Assurance & Regulatory Compliance
  • Ensure compliance with Minnesota Assisted Living regulations and company policies.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Conduct clinical audits to ensure quality care and accurate documentation.
  • Monitor medication administration practices and infection prevention protocols.
