• Provide compassionate and effective care and treatments to residents
• Work with a team of professionals to ensure comprehensive needs are met
• Review routines, calendar of events, and resident needs
• Ensure medications and supplies are ordered, in stock, and counted
• Maintain accurate and complete charting and record keeping
• Attend required meetings and trainings
• Promote the Mission, Vision and Values of Transforming Age and the Culture of Excellence
• Perform other duties as assigned

• Graduate of an accredited LPN Program
• Active LPN license in the state, in good standing
• Experience in a nursing home, assisted living or long‑term care preferred but not required
• Flexibility to work occasional weekends and holidays as needed
• Knowledge of DHS regulations
• Excellent interpersonal and communication skills; ability to work collaboratively with team members and external individuals
• Completion of background check, drug screen, and other pre‑employment screenings such as physical, TB test, fingerprinting per state/local guidelines
