What You'll Do
- Provide routine and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing settings.
- Complete nursing assessments, symptom management, medication education, and comfort-focused interventions.
- Support patients and families through education, advocacy, and compassionate communication.
- Collaborate with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
- Complete accurate, timely EMR documentation .
What You'll Bring
- Active Minnesota Registered Nurse (RN) license.
- Hospice experience preferred, or experience in home health, oncology, ICU, palliative care, med-surg, or another clinical setting with end-of-life care exposure.
- Excellent assessment, communication, and time-management skills.
- Ability to work independently in a field-based role.
- Reliable transportation and a valid driver's license.
