Case Manage and Responsibilities
  • Oversees CCD/ DD Waivered Service Deliver
  • Develops and reviews/ updates waiver recipients’ clients Community Support Plans Support Plan.
  • Arranges service planning annual meeting with the client, guardian, and providers.
  • Review Support Plans with the client, guardian, and providers annually and when services change. Update as needed/ required.
  • Provides ongoing (informal) review of all providers performance as well as formal semi- annual and annual performance reviews.
  • Meets twice a year annually with multidisciplinary team to assess validity and currency of Community Support Plans assess and supportive services that are in place to meet assessed needs and preferences of the waiver recipient. Updates the Support Plan with any changes.
  • Obtains diagnoses for individuals applying for HCBS services and annually for people receiving CCB Home and Community-Based (HCBS) waivered services.
  • Gathers input from client, guardian, and family members as well as providers to be used to determine eligibility for services.
  • Compiles appropriate written assessments and reassessments on clients.
  • Gathers and analyzes information to determine individuals’ status, established support systems, skills or lack of skills impacting community integration, and needs in each of the assessment areas.
  • Conducts MnChoices assessments/ reassessments for existing clients on an annual basis as requested.
  • To be knowledgeable of, remain current with and abide by the requirements set forth in Rule 65, CCB Waiver Rules and the Data Privacy Act.
  • Provide case management through a Person-Centered lens.
  • Follows the Person-Centered, Informed Choice and Transition Protocol.
  • Procures and monitors waivered and non-waivered services.
  • Locates and assesses available service provider(s) to ascertain whether they are qualified to provide appropriate services per the Community Support Plan.
  • Authorizes needed and appropriate services for the client, generate, implement, and maintain placement and service agreements per client support needs.
  • Provides early intervention and relocation service coordination activities.
  • Responds to individuals and family members requesting and assessing their need for HCBS and supporting them in accessing services that will support the individual avoid institutional services.
  • Provides assistance to residents and nursing home staff who are planning nursing home discharge. Identifies residents’ transitional needs and support them in acquiring and utilizing these needs.
  • Monitors and evaluates services to assure service provider objectives are meeting the clients CSP goals.
  • Actively participates in multidisciplinary team meetings semi-annually and annually or more often as determined by the CSP. Support Plan.
  • Reviews the providers records.
  • Determines whether client/ guardian are satisfied with services/ service delivery.
  • Compiles, reviews, and analyzes quarterly, semi-annual, and annual reports as determined by the CSP.
  • Revises CSP as required to ensure that it is revised as needed to meet CSP goals/ expectations.
  • Reports providers not in compliance with CSP and service plan expectations.
  • Arrange and monitor residential placements.
  • Completes placement agreements.
  • For children in foster care settings complete six-month administrative reviews and court review of placements every two years.
  • Reviews adult placements on an annual basis.
  • Performs general CCB/other case management duties.
  • Determines eligibility on new cases within 35 60working days of application.
  • Maintains all documentation as required in SSIS.
