Community Education Partner

Raising Health Partners (formerly known as Academy of Medical & Public Health Services (AMPHS)) is a Brooklyn-based, nonprofit organization dedicated to eliminating barriers to accessing healthcare for immigrants and other under-served communities in NYC. Every year, we proudly serve and connect more than 20,000 New Yorkers to healthcare systems. Our programs equip immigrant populations with the knowledge to help them navigate the complexities of living in a new city and transition into post-secondary education and/or employment opportunities.

At the same time, we also educate community members on their immigrant rights, healthy living, nutrition and health access, and create space for civic engagement.

We are looking for Community Education Partners (CEP) professionals to provide case management support and outreach helping Raising Health Educational programs. The CEP provides a collaborating compliment to the instruction, individualized counseling, and a nurturing environment for learners through our grant programs. CEPs provide key support to students for English for Speakers of Other Languages (ESOL) and other education programs, including career and college readiness, to assist a meaningful pathway toward qualitatively better lives and economic self-sufficiency.

Responsibilities include but are not limited to:

  • Acts as a confidential case manager for adult student issues, including assessing academic and personal needs and assisting students who may be in crisis or off-track grant specific program requirements.
  • Provides instructional support by assisting teachers with preparation, organization, and distribution of lesson materials and resources during classes. Facilitate classroom activities and addresses material needs as they arise.
  • Tracks and reports attendance information consistent with program requirements. Maintains accurate attendance records, follow-ups with absent students to support student retention.
  • Administers student assessments, analyzes assessment results, places students into appropriate proficiency levels, and maintains detailed assessment and placement records for student tracking, program reporting and compliance requirements.
  • Collaborates with instructors and program staff to monitor student progress, support student advancement and provide data for funder and grant reporting.
  • Screens and enrolls prospective students throughout the year as enrollment is ongoing. Attends community outreach events and represents Raising Health. Communicates with potential students, clients, and partners about Raising Health program offerings.
  • Tracks student progress, attendance, and satisfaction, while completing all data entry tasks as required by funders/grant programs. Ensures maintaining established Raising Health procedural and confidentiality measures.

Minimum qualifications:

  • High School Diploma required. Bachelor's degree highly preferred.
  • Bachelor's Degree in Counseling, Social Work, Education, or related field WITH two (2) years proven work experience in a nonprofit or social service organization providing case management and workshop facilitation or similar role required. OR A successful candidate with a High School diploma and 6 years of commensurate work experience described above.
  • Relevant experience in counseling individuals and/or leading workshops.
  • Strong awareness of the barriers facing immigrants looking to integrate into the US workforce and academic environment.
  • Experience supporting Chinese speaking populations in an advocacy or community support role. Understands the intricacies of working with a diverse range of people of varied socioeconomic backgrounds.
  • Ability to access and present resources needed to support student/client needs while out in the Sunset Park community.
  • Excellent command of the English Language.

Cantonese/English skills required.

  • Computer literacy required. Excellent command of MS Office Suite especially Word, Excel, and PowerPoint.
  • Ability to work Tuesday through Saturday from 10am – 6pm.
