Position Summary

We have an exciting opportunity to join our team as a Physical Therapist I - Brooklyn.

Responsibilities
  • In this role, evaluate physical therapy needs and implement comprehensive, medically prescribed physical therapy treatment to assigned patients in a supervised program of rehabilitation.
  • ASSESSMENT AND EVALUATION:
    Initial assessment and evaluation of physical therapy needs for specifically assigned patient populations. Review medical chart to ascertain important medical information and clarify referral/treatment orders if necessary for unfamiliar diagnoses. Select appropriate assessment tools and methods to accurately determine the patient’s status for developing an optimal treatment plan. Demonstrate effective clinical decision making and implement changes/adaptations to the treatment plan when appropriate.
  • CARE PLANNING:
    Develops and implements a care plan that meets the identified needs of the patient and care partners, including measurable short and long-term goals with time frames.
  • INTERVENTIONS:
    Implements treatment plans and protocols according to current professional and health system standards. Establishes achievable goals with patient and family. Considers a variety of validated treatment approaches and rationales for achievement of patient goals. Selects appropriate technical aids/assistive devices and correctly refers patients to other clinical services to achieve stated program/team treatment goals. Assesses educational needs of patient and families and implements methods for patient/family education and training during the rehabilitation process.
  • PATIENT PROGRESS:
    Evaluates patient’s progress and response to therapy, and modifies treatment interventions as needed.
  • EDUCATION AND SUPERVISION:
    Participates in clinical education of fieldwork students and other professional educational endeavors. Teaches and supervises assigned students in appropriate techniques, skills and procedures. Demonstrates genuine interest in assisting students and encourages positive learning experiences for Career Opportunity Students and other Non Fieldwork students.
  • ORGANIZATION:
    Maintains a level of patient care productivity as indicated by unit procedures and productivity reports.
  • DISCHARGE PLANNING:
    Develops and implements discharge plan in conjunction with patient, care partners and other members of the care team.
  • PARTICIPATION:
    Participates in team meetings, case conferences and committees as assigned.
  • DOCUMENTATION:
    Completes all required documentation for patient care and reimbursement in the medical chart according to Department guidelines and JACHO/CARF requirements, using appropriate medical and clinical terminology. Accurately records all treatment given, including daily treatment records.
Minimum Qualifications
  • Bachelor’s Degree in Physical Therapy.
  • New York State Physical Therapy License or temporary permit.
  • Good communication and interpersonal skills.
  • 0-1 years experience as a Physical Therapist.
  • New York State License in Physical Therapy.
  • CPR certification.
