Position: Counselor - Mandarin Speaking Preferred

Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program.

Position Summary

We have an exciting opportunity to join our team as a Counselor.

In this role, the successful candidate will serve as the counselor for the Virtual Primary Care project, responsible for screening patients for program eligibility and social drivers of health, providing education about Tyto Care to eligible patients, addressing identified social drivers of health, and assisting patients in accessing virtual health services.

Job Responsibilities
  • Conduct assessment to determine program eligibility
  • Screen for SDOH and address identified needs
  • Provide education and information about the Virtual Primary Care project and Tyto Care
  • Assist patients in using MyChart and uploading required documentation
  • Assist patients during virtual visits as needed
  • Screen patients for eligibility for resources and benefits
  • Assist clients with benefit applications
  • Make referrals to a variety of public and community agencies providing food, shelter, clothing, medical, educational, and other services
  • Assist community members in accessing health and behavioral health services within the FHC
  • Complete documentation in EPIC as required
  • Attend staff meetings and other required meetings
  • Counsel clients on available resources and provide advocacy
  • Strengthen relationships with community partners to identify resources for clients
Minimum Qualifications
  • High School Diploma or equivalent required
  • Bachelor's Degree preferred
  • 1+ years of relevant experience working, ability to plan and manage
  • Knowledge of public benefits
  • History of immigrant community engagement

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $41,137.15 - $48,000.00 annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
