Per Diem Nurse Assessor

Overview

Per Diem Nurse Assessor (Healthcare) — Consultant will complete the UAS assessment remotely. The consultant will review and fill out required forms at each assessment and upload data and all documents to Guiding Care System.

• Complete the UAS assessment remotely and review all required forms at the assessment.
• Upload data and all documents to Guiding Care System, including:
  Visit Acknowledgement Form;
  Informal Support Form;
  Caregiver And Doctor Information;
  Designated Representative Form; HIPAA Release Form;
  Enrollment Agreement;
  Service Consent;
  Home Safety Assessment;
  Home Visit Summary; PAH Education Provided; CDPAS Education & Forms; designated representative for CDPAS will need to have responsibilities reviewed at the assessment meeting.
• In cases where all documents were attempted but unsuccessful, comment on the checklist regarding the attempt.
• CDPAS Eligibility Form (for members receiving CDPAS services only) and All PCP/Specialist/Caregiver/Emergency contact information, including applicable NPI information;
  Time Tasking Tool (TTT).
• Use QAS Analyzer for all assessments and provide evidence of outcomes.
• Sign and finalize the UAS within 24 hours for the MAP plan and within 48 hours for the MLTC after go-ahead.
• Ensure UAS assessment is finalized on the same business day or within 24 hours of completion.
• Ensure all documentation is securely transmitted to the Plan with UAS finalized within 24 hours of assessment for MAP and 48 hours for MLTC, per NYS compliance regulations.
• Complete the Tasking Tool and upload documentation into Guiding Care; upload TTT.
• After the visit is completed, send activity to Intake.

Adhere to NYS compliance regulations and required timelines for finalizing UAS assessments and uploading documentation.
