ESSENTIAL DUTIES & RESPONSIBILITIES
  • Promotes the independence and self‑determination of individuals, utilizing person‑centered practices that focus on the individuals’ strengths and preferences while simultaneously addressing their needs.
  • Manages the holistic health care, nutrition, and wellness of individuals, including tracking appointments and communicating with CMS, nurses, relevant doctors and managers.
  • Ensures the coordination of all recreation and other community activities that promote the participation and independence of individuals.
  • Encourages the independence of individuals and promotes relationship building within the community and with natural supports.
  • Manages and directs the overall flow of the shift, including assigning staff to appropriate individual ratios.
  • Supervises staff (DSPs and shift supervisors) while promoting learning and development by providing continual coaching and mentoring.
  • Role models the necessary behaviors that support independence and the aspirational desires of the individuals.
  • Ensures that health and safety requirements are observed, including maintaining a stimulating and comfortable physical environment.
  • Facilitates individuals’ participation in recreational activities and medical appointments by coordinating all transportation activities.
  • Manages the personal property of individuals and their ability to access personal items.
  • Ensures compliance with regulatory agencies within mandatory time frames.

  • Additional responsibilities as deemed necessary by Program Director or designee.

Job Requirements

REQUIRED EDUCATION AND EXPERIENCE

  • Associate’s Degree in a related field and/or five years’ experience in /DD or related supervisory/management experience in a residential setting.
  • Effective written and oral communication skills.

PREFERRED QUALIFICATIONS & SKILLS

  • AMAP; SCIP‑R; CPR & First Aid (present, but not required).
  • Behavior Tech Training preferred.
  • Computer skills (e.g. MS Office suite and Windows).
