Core Responsibilities
  • Assist with patient oversight of Chronic Care Management (CCM), Physical and Rehabilitation Medicine (PRM), Behavioral Health Integration (BHI), and other POD programs
  • Monitor and review patient vital signs, alerts and other concerns
  • Answer phones, respond to inquiries and triage calls appropriately
  • Assist in the development and implementation of individualized care plans
  • Respond to urgent care needs when necessary
  • Assist with administrative tasks
  • Screen patients and make referrals accordingly to other clinical lines
  • Timely completion of clinical requests and follow ups
  • Documentation audit reviews
  • Maintain comprehensive medical records of all patient interactions
  • Perform other related duties as needed
Work Schedule:

Thursday, Friday, Saturday, Sunday and Monday 9am‑5pm

Required Qualifications:
  • Graduate from an accredited Licensed Practical Nursing program
  • High School Diploma or equivalent GED
  • Current Licensure and registration as an LPN in New York State
  • Excellent communication, interpersonal, organizational and time management skills
  • Ability to work independently and collaboratively within a team environment
  • Competency with EMR Systems and Documentation
  • Compassionate, patient‑focused, and committed to providing high‑quality care
  • Knowledge of complying with all state requirements and facility policies and procedures
Preferred Qualifications:
  • Current Licensure and registration as an LPN in New York State and Florida State
  • Spanish Speaking
