About the RoleThe Phoenix Rehabilitation and Nursing Center is seeking a compassionate and motivated Bilingual Social Worker (Spanish/English) to join our interdisciplinary care team. This role plays a vital part in supporting residents and families with psychosocial needs, care planning, and discharge coordination in a skilled nursing environment.
The ideal candidate is empathetic, culturally competent, well-organized, and passionate about advocating for residents.
Social Worker Bilingual (Spanish/English)Key Responsibilities
Conduct psychosocial assessments and develop care plans
Provide counseling and emotional support to residents and families
Serve as liaison between residents, families, and interdisciplinary team
Facilitate discharge planning and coordinate community resources
Participate in admissions and resident orientation
Address resident and family concerns and grievances
Maintain accurate documentation in the EMR system
Participate in care conferences and family meetings
Ensure compliance with CMS and state regulations
Bachelor’s or Master’s degree in Social Work (BSW or MSW)
Bilingual in Spanish and English (required)
At least 1 year experience in a healthcare or long-term care setting preferred
Strong communication, organizational, and interpersonal skills
Knowledge of discharge planning and community resources
Experience with Point Click Care or similar EMR preferred
Salary:
Up to $70,000 per year
(based on experience)
