Resident Care Liaison

Four Seasons Nursing and Rehabilitation Center is looking for a Resident Care Liaison & screener to join the team at our Skilled Nursing Facility in Brooklyn, NY.

Resident Care Liaison Job Duties and Requirements:

• Be able to move intermittently throughout the workday

• Be able to speak the English language in an understandable manner

• Be able to cope with the mental and emotional aspect of the position

• Be able to see and hear, or use prosthetics that will enable these senses to function adequately to assure that the requirements of this position can be fully met

• Function independently, have flexibility, personal integrity and the ability to work effectively with residents, personnel and support agencies

• Be in good general health and demonstrate emotional stability

• Be able to relate to and work with the ill, disabled, elderly, emotionally upset and at times hostile people within the facility

• Have basic computer knowledge and EMR

• Have weekday availability to cover 10AM-6PM

• Have availability to work every other weekend (once a month)

• Additional job duties and requirements as required by job description, Director of Social Services and/or Administrator
