Description:
Monitors patients for and provides education on food/drug interactions. Records food and drug interactions in the education section. Consults with physicians regarding appropriate diet modification. Screens and assesses patients in an accurate and timely manner based on levels of care. Determines patients' nutritional needs and status and provides follow-up care. Interviews patients and/or their families to obtain a history of food habits and preferences.
Interprets pertinent laboratory, physical, and social data in the medical record as it applies to nutrition care. Provides nutrition education and records patient's diet and diet instructions. Documents the patient's understanding of the diet in the education section. Modifies patient menus as needed.
