Job Summary:

Provides nutritional assessment, education, and dietary modifications for patients, ensuring food and drug interactions are managed appropriately. Collaborates with medical teams to develop and implement individualized nutrition plans.

Responsibilities:

• Screen and evaluate patients’ nutritional needs and health status promptly and accurately.
• Interview patients and their families to gather information on food habits, preferences, and medical history.
• Interpret laboratory results and clinical data to inform nutrition care strategies.
• Educate patients on dietary requirements and record their understanding and adherence to diet instructions.
• Modify and personalize patient menus based on medical conditions and nutritional needs.
• Participate in interdisciplinary team meetings and performance improvement initiatives to enhance patient care.

Qualifications:

• Hold a minimum of one year of experience in clinical nutrition or dietetics.
• Possess a degree in Dietetics, Foods and Nutrition, or related field from an accredited institution.
• Maintain required licensure, including New York State Certification as a “Dietitian-Nutritionist” or registration with CDR.
• Demonstrate proficiency with electronic medical record systems such as Point Click Care or Sigma Care.
• Preferred:
  Advanced degrees (Master’s) and additional certifications like AHA BLS.
