Role Overview :

As a key member of our post-acute care team, you will:
Provide comprehensive primary care services in short-term and long-term care settings Diagnose and manage chronic diseases and a variety of medical conditions Order and interpret diagnostic tests to guide treatment Prescribe medications and monitor treatment effectiveness Collaborate with physicians and healthcare providers to enhance patient outcomes Educate patients and families on health management and lifestyle changes Manage an average of 17 patients per day, fostering lasting patient relationships

Qualifications:

Minimum 1 year of primary care experience (post-acute care experience preferred) Active and unrestricted New York NP license Active and unrestricted DEA with prescriptive authority Compensation & Benefits:
