Location: Crown Heights, Brooklyn, NY
Setting: In-Office Telehealth with In-Home Medical Assistant Support
Compensation: $80?$100 per hour
Schedule: Sunday?

Thursday, either 2 PM?9 PM or 3 PM?10 PM (on-site only)

A fast-growing, innovative healthcare provider in Crown Heights is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to lead in-office telehealth visits for a diverse patient population. In this unique model, the provider works from the office while a trained Medical Assistant visits the patient?s home to assist with exams, vitals, and procedures.

• Conduct telehealth visits for patients of all ages, including family medicine, pediatrics, and geriatrics

• Evaluate and treat common conditions such as strep throat, flu, RSV, rashes, eye infections, and seasonal allergies

• Diagnose and manage ENT-related issues such as ear infections and sinus symptoms

• Coordinate with on-site Medical Assistants to assist with specimen collection (e.g., strep cultures, flu and COVID-19 testing), basic physical exams, and vitals in the patient?s home

• Ensure accurate charting and documentation in the EMR system

• Deliver compassionate, accessible, and patient-centered care

• Licensed NP or PA in New York State

• Clinical experience in family medicine, urgent care, or primary care settings

• Comfortable working with telehealth platforms and in-office telehealth workflows (this is not a remote position)

• Strong clinical judgment and ability to work independently while collaborating with a team

• Excellent communication and organizational skills
