The Bedford-Stuyvesant Family Health Center (BSFHC) is a patient-centered, team based multi-specialty practice that has a full-time opportunity available for a Pediatrician in central Brooklyn. BSFHC has been providing affordable, comprehensive, outpatient medical and dental services in an outpatient setting for over 40 years. As a Federally Qualified Health Center (FQHC), BSFHC serves all of the primary health care needs of families in the heart of North and Central Brooklyn.

We are seeking a Full-time Nurse Practitioner to join our team to provide high quality innovative care to patients and families in general pediatrics and will work closely with a team of professionals to provide a comprehensive patient-centered plan of care to achieve expected outcomes and BSFHC Mission.

Must be comfortable with both Pediatrics and Adult patients.

Job Responsibilities
  • Conduct age appropriate assessments of patients, including history taking and physical examination in accordance with current standards of professional practice, regulatory requirements and agency policies and procedures.
  • Perform assessment/data collection in an ongoing systematic manner, focusing on physiological, psychosocial, cultural, spiritual and cognitive status, patient/family education, family involvement, and patient advocacy.
  • Consult with other clinicians as dictated by patient need and assessment findings.
  • Participate with physicians and other clinicians in the department to assure effective patient care consistent with quality indicators for patients cared for in the designated service.
  • Review medical records for follow up or ordering of diagnostic studies under established protocols.
  • Provide primary medical care to patients and manage acute and chronic conditions.
  • Assess patients’ conditions and age-specific medical needs; set goals and prescribe appropriate clinical interventions to meet these goals.
  • Prescribe appropriate laboratory and diagnostic procedures related to patients’ health status and facilitate appropriate treatment intervention predicated on results of these procedures.
  • Provide comprehensive medical services for members of family, regardless of age or sex, on continuing basis: examine patients, using medical instruments and equipment.
  • Participate in quality assessment and improvement activities and achieve productivity goals. Examination of patients, formulation differential diagnostic plans, define and order required diagnostic testing.
  • Interpretation of examination findings and test results, and implementation of treatment plans.
  • Determine need for consultation and assist in medical care and treatment provided at the direction of other specialists.
  • Answer patient telephone consults with the assistance of clinic staff.
  • Order or execute various tests, analyses, and diagnostic images to provide information on patient’s condition.
  • Maintain accurate and detailed medical records about each patient, including observations during examinations (via EMR).
  • Administer or prescribe treatments and medications.
  • Promote health by advising patients concerning diet, hygiene, and methods for prevention of disease.
  • Write orders to inoculate and vaccinate patients to immunize patients from communicable diseases.
  • Refer patients to medical specialists for consultant services when necessary for patient’s well-being.
  • Work continually to assure delivery of quality clinical care in a safe, efficient, competent, and effective manner.
  • Prepare records and reports as required in support of services rendered in accordance with established procedures.
  • Attend and participate in patient care reports, patient care conferences, team conferences, professional staff conferences and other appropriate professional activities only to the extent that such attendance and participation is relative to his/her assigned cases and/or performance of services as determined by the Medical Director.
