Job Summary:
Provides essential patient care in a long-term care or skilled nursing facility setting, assisting residents with daily activities and maintaining their comfort and dignity.
Responsibilities:
- Perform bathing, showering, grooming, and hair care for residents according to schedule and needs.
- Assist residents with dressing, transfers, and mobility using appropriate lift devices and assistance protocols.
- Monitor and record vital signs, bowel and bladder functions, and fluid intake/output, reporting significant changes to nursing staff.
- Support residents with oral hygiene, denture care, and skin care, including incontinent care and repositioning to prevent skin breakdown.
- Assist with meal preparation, feeding, and providing access to hydration, ensuring nutritional needs are met.
- Document care provided, observations, and resident responses accurately in electronic medical records.
- Possess a high school diploma or GED.
- Complete at least 100 hours of approved nursing assistant training in a licensed facility.
- Hold a current New York Nurse Aide Certification and be listed on the New York State RHCF Nurse Aide Registry.
- Demonstrate ability to communicate compassionately and follow verbal and written instructions.
- Possess basic knowledge of infection control, residents' rights, and safety protocols.
